New Patient Process
1. Complete Health History Forms
Follow the link below to access our streamlined patient portal. You will be prompted to create an account. You can complete your health history forms directly from your account or if you prefer to download and print them you can return them via secured email to info@letsbegolden.com or drop them off at the office. We will not be able to schedule an appointment until the health history forms are completed and returned.
2. Next Steps
Once our team has verified your health history, we will reach out to schedule your first appointment.
